veritae on "[Plugin: Contact Form 7] One common form on separate pages - all working except one page"

This is the main page:

And these are the other pages that have the same form:

http://www.christadang.com/training/
http://www.christadang.com/about/how/

They all use the same shortcode for the same form created in CF7. When you hit "let's do this!", on the three pages, it works. But then on front page, nothing happens when the button is clicked. This is quite strange since it's the exact same form. What can I do?
