Could you tell us if you are using WordPress MU (multisite)?
This plugin is not compatible with WordPress Multisite.
If it is not WordPress MU, I would like to request you to email us the site access details so that we can have a look. Please create a test user with administrator role so that we can get access to the dashboard and have a look.
